- Purchase order number. If purchase orders are not used a comparable substitute is acceptable, i.e., invoice, encumbrance, or check number
- Purchase date
- Line item details (Detailed description of the purchase)
- Line item quantity
- Line item price
- Vendor ID number, name, and address
