As a regulatory authority, ORCAA relies on a number of forms designed to streamline processes and make it as simple as possible for the businesses and individuals to comply with clean air laws and regulations. Public Records Release Form

Individuals, journalists and businesses may request the release of any ORCAA materials deemd to be public records by submitting their request in write. Click here to find further information.

Complaint Form

Individuals who would like to file complaints may do so in person at the ORCAA business offices in Olympia or Port Angeles; they may call in their complaint (1-800-422-5623); or they may file a complaint online by clicking here .

If you'd rather, you may download a complaint form and mail or FAX it to ORCAA. The FORMAL Complaint Form is available here.

Land Clearing Burn Permit Application

Required for land clearing burns within ORCAA's jurisdiction.

Training Fire Permit Application

Required for any agency planning on burning a structure as a training exercise.

Asbestos Forms

• Residential Asbestos Permit
• Commercial Asbestos Permit
• Amendment To Asbestos Permit
• Asbestos Check-list for Demolition Projects

Must be completed before starting any project in which asbestos could be disturbed.

Demolition Permit Application

Must be completed before any structure demolition project is started.
